I have written a tutorial, about how to create your own HDRI and how to input it into your 3d application. Check it out if you want.

3DHuntsville : Tutorials

Just scroll down until you see the HDRI tutorial

It depends on the type of hdri image. if it sphereical then you want to use a sphere for the image, if it is a rect then use a rect, if it is a circle use a circle.

one you have got the shape the you need to apply the image. (i have not done these in max so i do not know how to apply it, I would just aply it as a texture. then make the image self illuminating.)

once your there the best thing to do is just play with the settings until you get the desired out come. playing around is the best way to learn because you see the changes and can start to understand what which controllers do.
